Privacy Policy

Privacy of your personal information is an essential part of providing you with quality service. All clinicians who work in this office are also private practitioners and as such have their own policies and procedures. Although I may work collaboratively on cases with some of these clinicians, you will need to sign a separate consent form allowing us to exchange information about you. The administrative support staff that comes into contact with your personal information have been trained in the appropriate uses and protection of your information and will adhere to the policies outlined below with respect to the handling of your information.

Collection of Information
In the course of your work with me, you will be asked to fill out a client information form and a service agreement. You will also provide me with information during our interview sessions. This information is collected for both clinical and administrative purposes.

Disclosure of Information
The information you provide to me will be kept confidential. This means that I will not willingly disclose information to anyone without your written consent except in the specific circumstances that are outlined in the service agreement.

In the event that third parties pay for the cost of the clinical services provided (e.g., insurance companies, Employee Assistance Programs), personal information may be shared. If you are receiving services under an Employee Assistance Program, information is not shared with your employer.

In Case of Emergency
Whenever possible, I will do my best to accommodate requests for emergency sessions if needed. However, I do not provide crisis intervention have an “on call” service available. Therefore, if you are in crisis, or require a more immediate response, please contact or go directly to the closest emergency department of your local hospital.

Use of Phone, E-mail and Fax for Transmission of Information
Phone messages may be left on my confidential voice mail or with administrative staff. Please be aware that it may take me up to 48 hours to return your call and that I have two office locations so messages may be left at either location.

Information of a sensitive or private nature is best communicated by phone or in person at sessions, to ensure the optimal amount of confidentiality can be provided to you. Should you request that information be transmitted by e-mail or fax, you will be asked for your written consent to do so. As such, you are agreeing to assume all responsibility for the transmission of the same and understand that information sent via email may be accessed by others during the transmission on the World Wide Web.

I reserve the right not to discuss private or sensitive information via email and will limit email transmissions to receiving your email to me, setting up appointment times and only other practical information exchanges rather than therapeutic discussions. Regular session fees apply to all communication or reviewing of documentation sent by fax or email.

Any email that is sent from this office automatically has the following message attached to it:
“This email, including any attachments, is intended only for the addressee named above. It contains information that is confidential or otherwise protected from use and disclosure. If you are not the intended recipient, please be advised that any review, disclosure, copying, or dissemination of this transmission or the taking of any action in reliance on its contents, or other use is strictly prohibited. If you have received this transmission in error, please notify the sender immediately and delete it. Thank you for your cooperation.”
